Yes, there are some out-of-pocket costs to each Member Household, including:
- The Primary Responsibility Amount (PRA) of $1,000 (65 years old and older only) $2,500, $5,000, $7,500 or $10,000 is the annual amount of personal responsibility that the Member Household is willing to assume before their Eligible Medical Bills can be shared by other Impact members.
- At the time of service, members pay a Provider Fee of $50 for each primary care visit, $75 for specialist or urgent care visits, and $150 for an emergency room visit or hospitalization. These fees are not applied to the PRA and are paid even after the member has met the PRA for the year.
- For Eligible Medical Bills, once the PRA is met, the individual Member Household will be responsible for a co-share of 10% up to the co-share limit of $5,000.
