How do I submit a claim?

Need information about reimbursement?

Members do not file claims, nor does Impact handle claims.

Since we are not an insurance company, when you go to the doctor, your provider submits your bill to Impact for discounting and processing.

If the member pays the provider directly, please refer to this article about the reimbursement process.  The instructions are available on the back of every Impact member card.

Impact's EDI Payer ID number is IH400. If necessary, paper HCFA or UB forms can be mailed to:

If your provider does not bill Impact directly, then you can obtain the HCFA-1500 form, UB-04 form or a Super/Itemized bill and submit it via a medical expense form. Medical Expense Form - Impact.
