Yes, there is an application fee when joining Impact.

When you join Impact Health Sharing, there’s a one-time application fee of $79. This fee can easily be paid through an electronic check or EFT, which you’ll set up during the application and account activation process. Just a quick heads-up: this application fee is non-refundable.
