Impact Health Sharing membership requirements
The Impact Health Sharing membership requirements include the following:- Members cannot use tobacco, vape, or use federally illegal drugs.
- Medical information is provided during the application process. If an applicant or a member fails to disclose medical history, lifestyle habits, or choices that may disqualify them from membership, and that information is discovered, medical bills will not be shared and/or their membership may be canceled.
- Applicant must have a U.S issued Social Security number and a valid Identification Card issued by the U.S. or Applicant must have a Individual Taxpayer Identification Number (ITIN) and also have a government issued Identification Card issued by Mexico, Canada, Guatemala, and/or the United States
- An adult child (18-25) must meet the same requirements in order to be on their parent's household membership and have their Eligible Medical Bills shared.
